Always visible in the app

Is this private, or is a care team connected?

Personal record mode: entries remain for the individual’s own reference unless they choose to export or share them. No clinician, practice or hospital is automatically watching, and pressing a concern button does not summon a clinical response.
Linked care pathway: a provider may configure the service for named teams, review queues, response hours and escalation routes. These arrangements must be explained locally; they are not assumed merely because the app is installed.

Check how you feel

Use the app only after considering the person’s overall condition. A photograph cannot show every serious problem.

Match the photograph

Follow the baseline outline and quality prompts for angle, light, focus and distance.

Add honest notes

Describe pain and other changes in your own words rather than trying to make a diagnosis.

Check the outcome

Confirm whether it is saved privately, awaiting review or has an agreed action.

Taking useful photographs

Consistency is more helpful than perfection

Use the same place, body position, camera orientation and light where reasonably possible. The app should help you reproduce the view.

  • Do not delay urgent care to take a photograph.
  • Do not touch, clean, squeeze, remove tissue from or dress a wound differently because of the app.
  • Do not include another person or private paperwork in the image.
  • Do not apply filters, beauty modes or manual editing.
  • Retake blurred, very dark, overexposed or obstructed photographs.

When you are concerned

Use clear words—and use the right urgent route

Tell the team

Say what has changed, when it changed and how the person feels. Include symptoms such as increasing pain, heat, swelling, discharge, smell, bleeding or feeling unwell.

Do not rely on messaging for emergencies

Call 999 for immediate danger or serious illness. Use NHS 111 or the provider’s urgent route when medical help is needed quickly. A message may not be read outside service hours.

A safe response tells you the action, time, location or contact route and what to do if the situation worsens.

The service should adapt to the person

Communication

Record preferred language, interpreter need, communication format and whether information is needed in Easy Read, large print, audio or another accessible format.

Support and consent

A carer or representative must use the correct proxy route. Capacity, consent and best-interest decisions remain governed by the care provider and applicable law.

Respect

Use the person’s name and respectful form of address. Ask only for sex, gender, religion or cultural information that has a clear purpose for care, safety or communication.

Your information

Know what is recorded and who controls it

Originals and derivatives

Your original image should remain intact. Any aligned view must be labelled and traceable to its source.

Sharing

Personal mode does not automatically share. Linked mode uses the provider’s configured lawful basis, permissions and privacy information.

Your rights

Production privacy information should explain access, correction, restriction, deletion where applicable, complaints and who to contact.
